The University of Nebraska Medical Center College of Nursing received a $100-thousand grant to develop a model on the care of older patients transitioning from a hospital to a skilled nursing facility.  Dr. Claudia Chaperon is an associate professor and principal investigator for the grant and says patient safety and care is their number one concern.

Clinical partners will participate in the clinical education of students.

The model will focus on assessing and evaluating complex patients.
